Forum outage - 14/02/2011

I was updating the code to resolve two oustanding issues which worked fine on my local copy:
- Top navigation bar not lining up on Firefox, Chrime and other browsers
- Quick reply button not showing at bottom of forum topics
For whatever reason, this broke the forum style and restoring the files I changed from my local backup didn't resolve it either, so I had to track down what the issue was which showed up several other errors which I've fixed too.
The original two issues should now be fixed as well

If there's something I've managed to overlook and looks different or not quite right, post it in this thread.
